BeAM (bedtime minus pre-breakfast glucose) in mg/dL from Zisman 2016. Provide bedtime_glucose_mg_dl or bedtime_glucose_mmol_l independently of AM, and am_glucose_mg_dl or am_glucose_mmol_l. mmol/L converts as mg/dL = mmol/L × 18.
