du Bois 2011 abbreviated four-predictor IPF score from age (<60 → 0, 60–69 → 4, ≥70 → 8), 24-week respiratory hospitalization (true → 14), FVC % predicted (<50 → 18, 51–65 → 13, 66–79 → 8, >80 → 0; 50 and 80 unpublished), and 24-week FVC % change (≤ −10 → 21, > −10 and < −4.9 → 10, ≥ −4.9 → 0). Returns a 0–61 total only. Does not interpolate Table 4 risk percents. Not GAP and not a transplant list
